Gains goal after illness: 30 lbs in 1 year

I developed liver disease, cirrohsis, and lost 40 lbs. The drugs that kept me alive also caused severe malnutrition. I have now had the rare, invaluable gift of a new liver transplant and discharged 16 days ago.
I have some energy now and stretch, meditate and walk 4K steps a day in addition to eating a surplus 500 cals a day. I've always had good diet, except for booze, an athletic body type and I want to make sure i put the right weight on in the right places. Targetting 15% fat. Being 54 y.o. makes it a bit tough but rehab is my full time job now.
