We chatted for a minute and then she asked me, "You've lost a lot of weight. I hope you didn't lose it for health reasons."
you're the one who was there, but just from seeing the words i took it to mean that she wanted to acknowledge the change while not wanting to be insensitive in case an illness was behind it.
if she was saying 'did you just decide to lose weight or did your doctor order you to?' then that's just plain rude.

some people lose weight as a result of illness so she is making sure you are fine.
