Two people standing side by side on an escalator so no one can pass

It's okay. I'm terrified of escalators and seldom get on them. I'm too busy clinging to the railing for dear life to try and walk up or down them. lol

Not slowing down for yellow lights and speeding up before they turn red?

Not ok and pretty dangerous. BTW I share your fear of escalators, which is why I like to walk up or down. As long as I look down and keep walking I'm ok. I hate standing there hanging on for dear life because I feel like I'm going to fall.
Asking your host at a house party to turn the music down a little because no one can talk

I think it's okay to suggest that the music be turned down a little.

Not okay. Unless they break both their hands or something and ask you to check it, then it would be okay. :)

Telling a friend at a business dinner that she's had too much to drink and is close to making a fool of herself

This is a tough one. It would seem like the nice thing to do would be to tell her, but it might make the situation worse if she's drunk. I'm going to say not okay.

Not "paying it forward" when the car in front of you pays for your order?

No - It's bad manners anywhere, and in the USA it is a Federal felony.

Knocking your spouse or roommate's toothbrush onto the bathroom floor, rinsing it off and putting it back in the holder without telling them.

Not ok. You don’t have to say anything bad about the food. Just no thanks, I’m not hungry is enough.

Being 5 or 10 minutes early to pick someone up.
