Why do I lose motivation or feel paralyzed to choose or eat right.

I try really hard and have a little success but then I start self sabotaging my my efforts why? I’m so frustrated.

Motivation can come and go like ocean waves…
No worries, shake it off and try/practice some more... you'll nail it and then you'll tackle the next things.
Trying and practicing are not good/bad kinds of things... learning and changing anything takes a strategy, try it, adjust it, try more. Rest and sleep, then try more tomorrow - to help yourself - that's what this is - helping ourself be healthier - hang in!

Motivation is a bad foundation for weight-loss, it's fleeting and not reliable. It's better to gradually build new habits. The expression 'try really hard' larmes me think you may be trying to make big changes rather than gradual ones? And potentially unpleasant ones or ones that you cannot keep up long term? Perhaps you are too rigid in what you consider 'eating right'?

Yeah the term “right” is standing out to me. For me it’s more about “how much” than right or wrong. For example, I just made myself a waffle with peanut butter for breakfast. Instead of “is peanut butter a right choice,” I threw the waffle on the food scale and chose 18 grams peanut butter instead of 25-30. In the evenings, instead of “ice cream would be so wrong / bad,” I grab a 100 calorie ice cream bar I’ve got ready in the freezer.

It sounds like you might rely on "will power," big changes, and motivation a lot. I can't do that. I had to start with small changes and keep adding them over time, for one thing.
Also, I have very little "will power," so I make sure I don't have to rely on it much. I order groceries and have them delivered—and I make my order right after I've eaten, and I use the previous order to remind me what I might need. I'm so much less likely to impulse buy, and when I don't have my trigger foods in the house, I'm so much less likely to go overboard on them. I have no problem popping out to get a single serving of something I want if I really want it. Fortunately for me, laziness trumps craving about 9 times out of 10 and I don't go.
I don't know if I'm very motivated or not. Yes, I really want to be healthier for a number of reasons, but that has never mattered in the 25 years I've been obese. I know the times I've lost weight down to "just" overweight, I got into slightly better habits. I've always relied too much on exercise, though, and this time, due to a medical issue, I can't exercise much, so I've had to lose by changing my eating habits more, and it's working. Keeping those new habits will be the new challenge for me.
