Do it first thing in the morning. Watch a motivational video or something that inspires you first. Then put the music on (the *kitten* kicking kind), get out the sheet that has your exercises on it and do it. Do it the same time every day. Start with the exercises you can do easily.
If that doesn't work, do some serious exploring of what could psychologically be holding you back.
I was in a similar situation recently and realized that, at the core of it, I just wasn't worth the effort.
I was not worth the effort of working out hard for my health and happiness.
When it came down to it... my efforts were already used up on others- their wants and needs- not mine.
I learned my worth or how to treat myself by others around me from day 1 to now.
Sometimes we have to un-learn things, and sort of re-program. Just an idea.
