That’s pretty unrealistic as you are already tiny.15

It's impossible to say how long it would take to lose a certain number of inches from a specific body area, weight comes off from all over, as it wants.7

It sounds like you're at a good place already2

Your BMI is towards the bottom of normal. Losing five inches around your waist could easily put you into dangerous territory.

Your organs will probably preclude you losing that much from your waist from where you are now. Uterus, bowels etc - they take up space. 26' is very small already. If you want your waist to LOOK smaller, I'd recommend building some muscle in your glutes and legs.13

Someone as petite as you are would probably not be able to trim 5" off the waist. There's simply not that much left to lose off your waistline. You might be able to tighten your stomach a bit more if you tone up with regular exercise. But I would bet that it'd be a subtler change than 3" or 5".
There have been very few adult women with naturally 21" waists. If you want a smaller look temporarily you might be able to achieve it with shapewear or a corset.8 -
you could remove some organs. i guess that would do it.20

But not right away, what with all the water retention from healing such a drastic surgery... And it would probably leave scars.13 -
If Mammy had only pulled a little harder on Scarlett’s corset and bustle, she could have saved 1000+ pages of angst.
Why do this to yourself?
Agree with the suggestion you look at weight training / recomp

In addition to the above comments: Why would you want your waist to be 21-23”?
Based on my quick internet search, your measurements are already on the low side. So, smaller could translate into unhealthy.
Also, your ribs are likely in the way of any waist reduction. Which means if you really wanted to reduce your waist you’d have to have a rib or two removed. Our ribs are there for a reason. I do not recommend this at all.

You're the same size as me I think. And almost impossible to have a waist that size unless you are naturally built that way.0

21” waist didn’t seem feasible to me. After a quick google search to even see what that would look like, everything seemed to show waist trainers. After looking through a handful of pictures that showed before/after shots (most said 26”-28” before), I must say that the before shots looked better. I’m not trying to sound judgmental, but the larger waist was still small but looked much nicer and natural to me. The super pinched 21” waist in all of them looked very unnatural and unflattering. It looked like they lived in a corset… which fair enough, that’s exactly how they got that size and shape.
I’d recommend some self reflection, learn to love yourself. You are already very petite (and young- you will see many more changes to your body through your 20s) and your measurements are fine! Don’t strive to look like super edited Insta content… you’ll need filters/editing for that!2 -
I’m looking down at my thighs. Each thigh is nearly 22 inches. The thought of someone wanting to get their waist down to a size lower than one of my legs freaks me out a bit, not going to lie. But then I’m a heck of a lot older than the OP, I’m not bombarded constantly by social media telling me I’m not good enough, and I now value my body for what it can do and lift rather than how it “should” look. It’s so sad that society puts this kind of pressure on people - OP you need to go out, have fun and rock the body you have. Please don’t be sucked into airbrushed pics or told you are not enough because you don’t fit an arbitrarily selected statistic x16

It would make more sense to increase the bust and hips2

I'm almost the same measurements as you except I'm 5'6. You have to understand, your colon takes up space. You have very little body fat to move out of the way. When I was severely underweight, my hips were 33, just pure bone there, no fat at all, and even then my waist, which literally just consisted of skin over colon material and measely bit of muscle, was still 24 inches. Hope you don't mind looking like a crack-head on death's door because that's what being that thin looks like to everyone else. Your face at this weight looks like a skull with a thin layer of skin over it.
However...
Another way of getting a bigger waist to hip ratio which will make your waist appear tinier is the build up your muscles both on the side of your hips, and your back (lats) and shoulder muscles.5 -
I'm 5'1" too! I am at 24-24.5 in waist right now (if I gain a bit of weight) with 36% body fat. I am hoping over the next few years to work hard on recomposition, which will likely reduce a few inches off my waist. I don't know if 21 inch waist is possible, and I am personally concerned with my waist size - but figuring out your body fat percent might help you get a sense of how much more fat you could may be lose off your waist. For me, I also loose the same inches off bust and thighs at the same time.0
