Cate, i think research has been done over and over and not shown conclusively to do any of those things.
In my personal experience it certainly didnt cause weight gain and my last blood tests were within normal fasting glucose range so no evidence of causing insulin resistance either.
I dont think anyone is arguing that it is healthy - as in provides nutritous benifits - but that is ok. Sometimes i just consume foods/drinks because I enjoy them, and as part of an overall balanced diet that is fine.
I dont eat chocolate because it is healthy either (yes I know some arguements can be made for health benifits, but that isnt my purpose in consuming it)
Of course not if the product is actually harmful - but something can be 'not healthy' without being harmful

It can not physically lead to weight gain. And if it promoted insulin resistance it wouldn't be recommended to diabetics.

And wouldn’t water, or kombucha, or fresh squeezed kale juice be a healthier choice than black coffee?
There’s always going to be a healthier/better choice of what to consume - how to live your life - how to spend your money.... if you always strive for optimal, no one would ever buy a frivolous pair of shoes because wouldn’t it be better to save that money? No one would ever get ice cream with their kids on a hot summer day because isn’t a banana healthier than ice cream? What kind of life doesn’t include fun shoes and ice cream on occasion?
Life is about balance and finding a combination of things that improve your overall health and quality of life and that includes mental health and personal enjoyment. Not everything has to add health benefits in order to justify consuming it. Diet soda has proved time and again to be safe for consumption. It doesn’t have calories so it is “better” on that front than an alternative calorie laden beverage if someone is trying to restrict calories. Even compared to black coffee, if a person enjoys diet soda and it isn’t harmful, why shouldn’t they choose diet soda?

Since I hate coffee and love sodas, it’s Diet Pepsi for the win. Triple score for being diabetic.
Coffee has many times the amount of caffeine and if I drank that vile stuff I’d be up all night. Besides the water doesn’t sizzle.4 -

My doctor has actually gotten after me about coffee-it has a lot of caffeine in it, compared to a can of diet soda. When I go in for my annual there's a form I have to fill out and it asks you how much coffee, tea, soda you drink every day (you assign a number by each). I always get comments about my cups of daily coffee but he's never said anything about my cans of daily diet soda (also, for what it's worth-my doctor is an MD who came from a sports medicine/nutrition background, before switching over to general practice, so he's had more nutrition education than the typical MD).5 -
Frankly I don't give a "Hoover Dam" about the studies, one week "they" say something will kill you, then another week it's good for you. If I like it, I'll eat it or drink it. Life is to dang short to worry about what may or may not kill us, or what may be bad for us.
I stopped drinking soda, diet or otherwise on a regular basis unless it has rum in it, just because I felt like doing so. But coffee is my life blood and I will only give it up when I'm dead.
I suppose the main point should be, "all things in moderation."

Well no...studies rarely if ever show or state such broad strokes. This is the part of science journalism that pushes ideas well beyond the body of evidence to support confirmation bias or push an agenda. This is true for most scientific studies, all of which are far closer to "all things in moderation" than "you will die if you drink coffee".

Honestly, I feel better drinking water or black tea over any soda. Even if diet soda isn’t harmful it certainly wouldn’t qualify as healthy. At least coffee has antioxidants.

Aspartame has been well studied over the past forty years and we know exactly how it is broken down in the body. There is zero risk of it causing insulin resistance or weight gain.

No offense, but I take medical advice from my physician, not some stranger on the internet. And perhaps you’re correct and I should find a new MD, but that doesn’t make Diet Coke or any soda healthy. I stand by my opinion that black coffee is the better option.
I can almost guarantee you that that stranger (who has a PhD in a relevant field) has more knowledge about how aspartame works than your doctor.
